The risks for other pandemics in the future are expected to increase significantly because of climate change. The pandemic has also taught us how lives can be saved by handwashing at critical times with soap and water. Infection prevention and control (IPC), particularly in healthcare facility settings, is of utmost importance to containing and controlling the spreading of infectious diseases and antimicrobial resistance. The latter is contributing to 5 million deaths per year especially in low and middle-income countries[1].

In India, the world’s most populous country and a climate change hotspot, the lack of adequate water, sanitation and hygiene (WASH) services in healthcare facilities (HCFs) is creating environments where harmful bacteria thrive. Tdh India with support from SWSC, implemented a WASH in HCFs project in the climate-sensitive and geographically challenging Sundarbans region.

HCFs in the region lacked adequate WASH services including hand hygiene facilities. Therefore, medical service providers were facing challenges in washing their hands at points of care. Inadequate sanitation facilities and missing handwashing facilities for staff and patients were contributing to repeated disease outbreaks. They were often treated with antibiotics which in turn contributed to an increase in antibiotic resistance. Tdh India worked closely with government authorities from the beginning of the project by supporting the national Kayakalp programme initiated by the Government of India for maintaining cleanliness in HCFs. The project supported construction/renovation of WASH facilities and involved HCF authorities in the WASH FIT cycle. The main challenge was to ensure the operation and maintenance of improved WASH facilities for sustainability of the project interventions.

The Tdh India team has been successful in advocating with the government at different levels for sustaining improved WASH facilities constructed/renovated under the project. Local government authorities (Gram Panchayats) have assumed the responsibility for the operation and maintenance of WASH facilities in HCFs under their jurisdiction through advocacy initiated by the project. This commitment has been documented in the meeting resolutions of Gram Panchayats. Accordingly, they have allocated human resources in two HCFs which had shortages of cleaning service providers.

The film from Tdh India (in English)

Moreover, local authorities have allocated financial resources to support the operation and maintenance in HCFs in Gram Panchayat Development Plan (GPDP) for financial year 2023-24 and submitted to State authority for approval.
